Serious Case Review
This is a standing sub group and is responsible for auditing outcomes of all Serious Case Reviews.
Looking at cases where either a Serious Case Review or Independent Management Review is required in circumstances where there are lessons to be learned on significant harm issues (Working Together 2010) and making recommendations to the Chair of WSCB.
A Serious Case Review is when a child is subject to significant harm or dies and abuse, neglect are known or suspected to be a factor in the death. A new criterion has been introduced where a death could be of media interest.
The Group will ensure that a thorough review is undertaken and any lessons to be learned are understood and acted upon.
